Description
While there is an increasing evidence base in the treatment of patients with a diagnosis of Borderline Personality Disorder, this is not the case in Narcissistic Personality Disorder. The nature of narcissism suggests that the assessment, preparation for treatment and then the actual treatment is done best by being attuned to the particular difficulties that patients with the qualities or narcissism present. But what are these attunements and how does it reflect an analytic stance and technique? We will address some of these questions in this series of seminars.
DeanCross PD service (RCPsych team of the year in 2019) has an increasing experience in working with patients with NPD. The service has its basis in a psychoanalytic approach implemented through two streams in our programme, Mentalisation Based Treatment (MBT) and Transference Focused Psychotherapy (TFP). We have adopted a TFP approach to NPD and run a group with patients with narcissism as their primary diagnosis. We are offering a series of seminars which describe how we work with patients with narcissism in order to help clinicians at a practical level in their work with these patients. The seminar will introduce the concepts, strategies, tactics and technique of TFP, contextualising this within the body of psychoanalytic thinking.
This series of seminars is part of the wider Transference Focused Psychotherapy-UK training programme.
Aim
To describe the practical application of TFP in clinical work with narcissistic patients, to help clinicians at a practical level to work with these patients.
Objectives
- To describe a current understanding of Narcissistic PD
- To describe key tenets of the strategies, tactics and technique of TFP.
- To describe key aspect in the assessment of patients with a diagnosis of NPD
- To describe key aspects of the preparation of patients with a diagnosis of NPD for therapy
- To describe key technical modifications and challenges in group therapy of patients with a diagnosis of NPD
- The overall objective is that clinicians attending will emerge from the seminars with increased knowledge and appreciation of a practical clinical approach and technique in working with patients with a diagnosis of NPD.
Target audience:
Clinicians assessing or treating patients with moderate to severe personality disorder, including a diagnosis of Narcissistic PD, or with strong traits of Narcissism. It will suit clinicians interested in the clinical presentation of narcissism, and who would like an introduction to, or further familiarisation with TFP, an operationalised evidence based psychodynamic approach. In the nature of 4 seminars, participants will not achieve technical competence in TFP, however the aim is that participants appreciate the key tenets of TFP sufficiently to both critically appraise their current practice and to consider some aspects of TFP which they may consider incorporating into their current approach.

Seminars: All seminars are 14:00-17:00 (British Summer time on 6 Sept and 4 Oct, and GMT on 1 Nov and 6 Dec. British Summer Time is 1 hour ahead of Greenwich Mean Time)

Patients with Narcissism present particular technical difficulties in the need to develop a clear shared formulation of the psychological problem. This session will particularly focus on issues of group therapy, and the delicate balance needed to attend to group dynamics when the individual patient is highly sensitive to whether they are sufficiently attended to.
